oaire.objetivesSpecific Aims : Aim 1. To determine the age-specific prevalence of parasites in blood, mucosa and dermal aspirates of the inhabitants of foci of domestic transmission of leishmaniasis. Research Questions ' What is the prevalence and relative burden of Leishmania in extralesional tissues of symptomatic and asymptomatic populations experiencing domestic transmission of dermal leishmaniasis? ' Is there an age, gender or clinical status bias in the prevalence and burden of parasites indicating that some individuals may be contributing more to transmission? Indicators ' Age specific prevalence of parasites in tissues of clinically apparent and subclinical infection established ' Individuals with high parasite burden identified Implementable outcomes 1. Magnitude of human reservoir of Leishmania infection in setting of domestic transmission estimated. 2. Quantitative reference point for assessment of subsequent interventions to reduce the burden of human infection determined. 3. Target populations for host directed interventions identified.en

To discern the relationship between immunological evidence of infection (DTH response to intradermal application of Leishmania antigen-leishmanin) and the presence of parasites in host tissues (blood, mucosa and healthy skin). Research Questions ' Is skin test response correlated with the detectable presence of parasites in tissues? ' Can subclinical infection in the community be parasitologically established using molecular methods? Indicators ' Relationship between skin test response and persistence of parasites established. ' Capacity of molecular amplification of Leishmania DNA to parasitologically detect subclinical infection determined. Implementable outcome 1. A gene-based tool to parasitologically detect asymptomatic infection.en

Research Questions ' Are multiple populations of Leishmania distinguishable in domestic settings of transmission? ' Can the epidemiologic relationship of human infection with domestic transmission be discerned using phenotypic and genotypic markers? Indicators ' Diversity/similarity of Leishmania populations in representative settings of domestic transmission determined. ' Potential of phenotypic and genetic markers to discern epidemiologic relationships in the domestic setting defined. Implementable outcomes 1. Novel epidemiologic tools based on genotypic and functional markers 2. Genetic and phenotypic evidence of common or diverse sources of infection in the human population.en

' Is the parasite burden or drug susceptibility of recovered parasites correlated with the clinical response to treatment? Indicators ' In vitro drug susceptibility of parasites that disseminate to extralesional tissues and persist following treatment determined.
